Job Description:

Medical Case Manager – HealthWorks of Illinois

Part-time

Responsible for compiling and maintaining a complete record of each foster child’s medical history. Serves as a healthcare advocate for the foster care child and the foster family. Maintains regular contact with foster parents to provide support, information, and resources.

REQUIREMENTS:

  • Good communication skills.
  • Good computer skills.
  • RN or Social Work degree.
  • Detail oriented.
  • Must be able to sit, stand, and walk 70-80% of the time.
  • Must be able to stoop/kneel to file and retrieve files.
